Q: hey guys, well just wanted to say i am very pleased with your site. secondly, i need help. i am getting breast reduction done in less then a month, and i know i need it, i'm 5'4 124lbs and have a 34dd boobs, i'm going down to a 32c but i'm nervous, i'd like to know what to expect and i want to talk to people without being face to face. I want to make sure that my boobs will stay small like that. There's always to much of a good thing!! help. thanks kellA: Hi Kell, There is no guarantee that your breasts will remain smaller. If you gain weight and have a tendency to put on size in your breasts, they will grow. You must remember this as you manage your weight and overall health after surgery. I have large breasts, always been more than a DD but this year I'm turning 40 and I want to know if it's possible to have a reduction and then get implants. I love my figure but the weight seems to be pulling the skin on my chest. I don't want them lifted because I understand that doesn't always work well. But could the skin be removed permanently? And would my breasts "sit up and stay" for many years to come.....with or without implants....? I thought maybe having some if the excess fat and tissue removed along with lifting them might be better but I am not opposed to implants! I'm open for anything! Please help!! I know that with weight gain my breasts get larger so implants wouldn't make much sense.....also, what's the average cost for a lift or reduction or combo!! Thank you so much! Malinda A: Hi Malinda, Most surgeons advise against reduction followed by implants. This is because the weight of the implants can cause havoc with the scarring left by the reduction. Also, in cases where women have enough natural tissue, most surgeons will recommend a conservative reduction and lift to provide an organic look and feel. This is my recommendation as well for most women. I can not comment on "average" cost of anything, since prices range so drastically market to market and doctor to doctor. I advise consulting with a few surgeons in your area for prices and options. Best, Eve Q: I am 17 years old, I'm 5'0 and weigh 105. I am a small girl and to everyone my body is very proportional with a lil extra on my backside which isn't a bad thing. But my boobs have not grown since my freshman year they look like a 13 year old girl's. All my friends, even the ones with small boobs are normal looking while i have large areolas and they are cone shaped. It's embarrassing and and I can't stand it. I was on birth control for 5 months and they didn't develop then either. Is there anything I can do? Is there something wrong with me? However I'm 21 years old and am embarrassed to talk to my parents or anyone about having it removed. How do I go about getting it removed? What type of doctor would I have to go to? Is it expensive to get it removed because I don't have insurance so I was wondering around how much it would be. Also, what exactly do they do to remove it? Do they laser it off? I'm just curious about the entire procedure because I really want to have it removed. Kelly A: Hi Kelly, The procedure will depend on the nipple itself. If it is simply a surface feature, removal should be easy. If the tissue extends into the breast, it may be more complicated. Either way, removal will not be cheap, but it should not break the bank. I know it's very common for girls to have uneven breasts, but mine are almost 3 cup sizes different! It's a constant source of embarrassment, restricts my love life as I'm incredibly shy about it, as well as the fact that I cannot get any bras or clothes to fit. I'm not overweight, but curvy, like bum and thighs. One is a 36A, the other a 36C, almost D. I've looked at things like hypoplasia, but not sure if they apply because I appear to have some glands in the smaller one. And please don't ask me to see my doctor- I have already seen 3. They ignore me because of my age (I'm 17), telling me that they'll even up with time. But they're so very different! The bigger one has been growing since i was 12, whereas the smaller stopped at 13. Unfortunately, the doctors can not do much, due to your age. They might even out, they may not... However, it is too early to consider augmentation. You might consider a less drastic option, such as the BRAVA system. This might help to minimize the difference in the meantime until you even out or can think about other options. Skin excision is the name of the procedure which can remove the skin from the upper and underarm area. With such a large chest, you will likely require a full breast lift, utilizing a vertical or anchor incision. My natural breasts still have a good shape with a D cup. However, age has made them a little less full. One breast has a scar and is more a C cup because of a breast infection I had in my late 30's when I was breastfeeding. I have an interest in an augmentation to just equalize and return the fuller volume I had in my mid 30's. I don't need my nipples moved up. Just need to perk up my natural body. No one writes about this due to age. Is this done? Katie A: Hi Katie, Sure, many women consider breast augmentation for this reason specifically. I'm a 32AA on my left and probably 32AAA on my right breast (wear 32AA but don't fill out on my right). I don't want to get surgery but I hate not having breasts. It drives me crazy to even wear a bra (when I can find one) because I don't fill it out so it rubs weird. I am thin and small framed, 5'2" 98 lbs, and I've tried to gain some weight to see if it would help but don't gain weight easily. I've been up to about 105 lbs before but I still was a 32AA. I've wanted to try things in the past but have been scared that I'll damage my body. I'm not sure if there is anything all that safe that I could do. What would be your advice for someone my size? (or lack of :( Andrea A: Hi Andrea, The best recommendation I have for you is to consider the BRAVA system of nonsurgical enlargement. This will not make your breasts large, but should add a modest amount of tissue which can make a big difference on a tiny framed woman like yourself. However, if you are that concerned with the difference, I advise you get it checked by your doctor who can offer personalized recommendations. My current insurance won't cover any elective surgeries or complications arising from elective surgeries. I'm pretty sure there's no insurance out there to cover the procedure itself, but if you know of any elective surgery complication insurance for this kind of surgery, I would greatly appreciate it. Bill A: Hi Bill, No, I do not know of any. Unfortunately, any elective procedure and all events which follow are considered untouchables by the insurance industry. It is amazing how the companies will pay hundreds of thousands for often ridiculous and meaningless diagnostic tests and ineffective procedures, but will not help a person to achieve their lifelong goals mind/body and spirit through plastic surgery. Contracture is one thing and does not require immediate (or any) treatment in many cases. Rupture or leakage are other matters. I always advise patients with these problems to seek care immediately. Best, Eve Q: hi there, I'm 20 years old and have large breasts since i can remember. I've also had back problems since i was 12 (I've had 36DD breasts since then). My family and chiropractors have all suggested I look into breast reduction. HOWEVER...I am extremely uncomfortable with the thought of my nipples being removed and reapplied. I was hoping I could have scarless reduction surgery, but I also don't want saggy breasts. To get to the point... is it possible to get a scarless reduction and then an augmentation to be a size 36C? or is that just unheard of? Taylor A: Hi Taylor, The vast majority of doctors do not recommend a reduction followed by an augmentation. There are many reasons for this, but the most important being the implants will degenerate the scars used for reduction in most cases. You can get a reduction and lift for a beautiful result, although there is no such thing as scar less reduction. I would recommend researching vertical reduction, is this is an option for you. Most often, the nipples to not have to be removed, but in some cases, they may. This is certainly a difficult decision to make for most women, so take your time and learn all you can! Best, Eve Q: i had a double mastectomy last year with a trans flap reconstruction. My left breast seems to have healed just fine. The right one however seems to have developed an indent above the breast and now the right breast is at least 1 1/2 to 2 inches longer than the left. The right breast is also sore above and on the side. I have mentioned this to my plastic surgeon, and he says it is normal. I am becoming more self conscious about the dent, pulling and now misshaped breast. I am scheduled for a nipple reconstruction next month and asked the surgeon if he could fix the issue at the same time. He said that the dent is not repairable and is normal. Is there something else I can be requesting within my rights? Debbie A: Hi Debbie, Sure you have the right to a second opinion, which I highly recommend. It sounds like this doctor is not the right one for you... Mary A: Hi Mary, The only moderately effective means of breast reduction without surgery are diet and exercise. These modalities will not work for every women, but may work well for some. Best, Eve Comment: Mri has confirmed rupture and I saw a reconstructive surgeon that put me on their emergency list for consultation in Indianapolis on 2/9/10. They are discouraging me from having the explantation surgery and want me to do replacement. As of now they want me to pay up front with no insurance involvement just to get on their surgery schedule. My insurance tells me they will pay some of it if a precertification letter is submitted, which the surgery scheduler finally did on 2/12/10. She still has not sent me a price for explantation, as she said she would email to me. I am sitting here on Valentines Day with a bladder infection and out of control blood pressure and in pain, pain, pain and wondering what is next,because of that almighty dollar and our out of control health care system. It is not the insurance companies that is the problem, it is the out of control prices the doctors and hospitals want to charge the insurance companies for every little thing. I am a 10A and i am reasonably tall and thin but i have a rounder curved bum and i have been told i am 'bottom heavy' by my family for years. It's not 'big' but my boobs are out of proportion to the rest of my body. i know i am only young but i have dreamed of this procedure since as long as i can remember. i have come to terms with the fact that my breasts will not get any larger they haven't for 4 years so why would they now, i have been on the pill and off the pill and still no change, i don't get weight gain or loss and when i do even slightly my boobs don't change. i am sick and tired of wearing push up bra's and not wearing a bikini even though everyone i know says i have the perfect body for it. i'm confused eve is this the procedure for me? Megan A: Hi Megan, It may be, someday. Now, I advise that you are still too young to consider it. Although many surgeons will perform the procedure at 18, I always advise waiting till 21 or older. This is for 2 reasons... First, breasts can grow well into the mid 20's, which I see often. Second, you need emotional maturity to make this type of life changing decision. Unfortunately, there is no cookie cutter answer. Implants take time to look more natural and some never do "drop" . I always recommend speaking to your doctor directly for a timeline of what to expect and when to expect it, since they are intimately aware of the exact details involved. Susan A: Hi Susan, There are many doctors with experience working with large sized implants. There is also a steady demand for them. Unfortunately, the major manufacturers cap the sizes and it can be difficult to get all varieties of implant in ultra large sizes. For more info on the type of implants available to you, I advise consulting with a surgeon who is wiling to perform such an augmentation. Best, Eve Q: I've had a double mastectomy due to breast cancer, the results were amazing but shortly afterwards i needed a capsulectomy. Every thing went great, i feel good, but now six months later my left breast has contracted a tad, i can notice it but doesn't seem to be contracting further at this point (the last 2 months). My doctor wants to do an in office under sedation capsulotomy in office. He says its much less invasive, doesn't have to remove the implant. I just would like some input on this procedure. Perri A: Hi Perri, Statistically, full capsule removal is the most effective and permanent capsular contracture surgery. A very beautiful Lady, speaks and writes English well. Nattaya has a concern for having Breast Augmentation Surgery done in the United States. Her concern is communications with staff and doctors. Her English is vary good and our friends always tells her so. We have read the forums and discovered you are familiar with doctors etc in Thailand. Can you suggest a few vary qualified surgeons in Thailand to do breast augmentation? We are looking at surgeons in the Denver area as well. However I feel it is very important for her to have a lot of confidence in the doctor and staff. My concerns about having the procedure done in Thailand are, Doctors ability to do the surgery with minimum incisions and doing the implants correctly, if they will be able to do locals for pain management. Some doctors are using local instead of general in the Denver area.mI am concerned about the company reputation, guarantee on the implant and type of implants to use that are available in Thailand? After reading the forums it appears that cohesive silicone gel feels more natural with less risk for Nattaya than saline implants, Am I correct? They will be able to refer you to the best surgeon for your needs. I do not have any recommended contacts in Denver, but your local medical association or hospitals should be able to provide some good candidates. The quality of the work in Thailand is very high, as long as you go to a reputable doctor. AS far as implant choice, this is very personal. I advise talking to your doctor about this concern once you have selected a surgeon. Each has pros and cons, so you will have to see and feel each to determine the best for your needs. You need to begin with a comprehensive exam from a qualified physician. If you suspect that large breasts are playing a role your pain, then be sure to talk to your doctor about this possibility. Sindy A: Hi Sindy, At 19, and with your reasons for considering surgery, I certainly do not recommend it. Just because your friends are bigger means nothing and not all men prefer big breasts. In fact, as you age, you may be much happier that they are proportionate. Rather than even thinking about implants, why not consider nonsurgical enhancement, such as that offered by the Brava system. This way, there is no risk involved. If you still want implants in the future, do it FOR YOU, not for the perceived desires of "some theoretical guy"... Best, Eve Q: I had my surgery 13 years ago. The implants are saline & were placed under the muscle. My right implant ruptured a couple days ago. My life is very different now than it was 13 years ago and it's financially impossible for me to have surgery right now. Are there any risks involved in leaving a ruptured saline implant in place? If so, is there anything I can do to minimize the risks? Lisa A: Hi Lisa, Your implants should be under warranty. I strongly advise you to look into this, as it may pay some or all of your replacement costs. Yes, leaving a ruptured saline implant is very dangerous, as a sharp edge of the shell could damage internal tissues or even wear through the skin and become exposed. In the last few months my breasts have become notably larger and painful. They also tingle almost as though I had milk in them. I was bulimic then anorexic over about a 2 or 3 year period, I'm happy I have some meat on me now!! Mickey A: Hi Mickey, You have the choice common to many women who are unhappy about the size and/or shape of their breasts. Do you leave them alone and learn to live with them or do you change them? This is the question. Of course, no one can answer this question but you... Luckily, you are very young and still have time to consider and decide. I would like to fit into women's clothing. I hate sharing my shirts with my ten year old daughter. I've always hated how I look. It affects me socially as well. I'm not wanted to be the next "Dolly Parton", I just want to look like a woman. Here's the problem. I do not have the finances, as raising five children does not allow for extra cash. If you know of a surgeon willing to do this for free, I'd be willing to do almost anything at this point. Including having it filmed. The closest city to me is Ottawa. Best, Eve Q: I had breast augmentation initially in 1997 and I went from a AA cup to a B cup. I went on to have 2 children quite close together and breast fed them both with no difficulties. However when my 2nd child was 6 months old I developed capsular contracture (both breasts) they said grade 3 so decided to have them replaced, which I did in 2004. My breasts have been fine since, I have no changes to size or shape. I went on to have another child in 2007 and although I again breast fed It was more difficult this time as I suffered from blocked milk ducts. I have a loose bit of skin under my right breast which I had checked out. It was due to weight gain during pregnancy but they said if it bothered me I could have the breast done again, but it didn't so I left it. A week ago I discovered a lump in my right breast, it felt mobile but a little painful. I went to my GP who said it felt like a lymph node. He also said he could feel one in my armpit. He referred me to a surgeon at my local hospital just to check it out. I'm actually a nurse there so the next day I asked a colleague if I could be seen in the breast clinic. I went yesterday. She confirmed that there was a lump and that yes it felt like a lymph node. I went on to have a mammogram, which was normal, and then an ultrasound. As soon as the sonographer scanned my breast he informed me that I have a rupture in my implant and that silicone had leaked into my lymph nodes!!!! I can't tell you how worried I am. I can't get to see my surgeon until mid January and I'm scared to death that the silicone can migrate and cause further problems. I am seeking reassurance that this is ok and can be dealt with. The consultant at the hospital has referred me for an urgent MRI to see how far it has spread but I really don't want to go as I'm scared its further than they think. Apart from my breast feeling softer than the other, which it has done for a while now, it hasn't changed shape or size. Please help, Gayle A: Hi Gayle, I also advise that an MRI is a good idea. Usually, migration is limited and can be contained and corrected. Just do not wait any longer... Make sure that if you replace the implants again, that you choose saline or cohesive silicone gel, as these new silicone models can not leak or rupture. I went under the breast and got a 450 CC Saline implant. As time went by I started feeling pressure on my Left breast, I contacted my Doctor a couple of times and went in and he said it was normal it was probably my breast pushing on a nerve. Some more time passed and i noticed that my Left breast was slightly higher and when I moved a certain way you could see the shape of the breast. All I can tell you is that the implant you choose will look about 30% smaller inside your body, so keep that in mind when you are trying various sizes in your bra. As far as contracture, patients who experience it once may experience it again. However, many patients can have it successfully corrected with revision surgery. You can always have the implants removed, but then you will require an invasive lift to repair the stretch marks and loose skin. Of course, this will leave scarring, as well. Dana A: Hi Dana, You are basically describing 2 different procedures. Most surgeons do not recommend reductions with implants, as there is plenty of natural tissue to go around to re-form the breast in a smaller and lifted shape. Preventative mastectomy is a different story all together. This will remove all the breast tissue and you will need a complete restorative breast reconstruction to recreate the breast structure. This is incredibly invasive and is not likely to provide an aesthetic appearance. However, in some cases, it is indicated. I recommend you consult with a plastic surgeon for detailed advice on which is the best path for you. Paula A: Hi Paula, I can tell you are very upset by this turn of events, but you need to relax and stay focused on attending to the issue at hand, which is the leak. The implant should be replaced ASAP. I can not answer the questions you have about insurance coverage, since I do not know what type of implants you have and whether or not they have any warranty in effect (unlikely given their age). Additionally, I do not know the details of your health plan. I advise you contact your insurance company for answers to these questions. You do not want to ignore the leak. You can find a good doctor by interviewing several and getting their thoughts on the best solution. You need to find a doctor you are comfortable with and one who can meet your expectations in every criteria. Additionally, you should research the surgeon and make sure they do not have a history of patient complaints, or disciplinary actions. I have had to have my silicone implants replaced 5 times over a 30 year period and invested thousands of dollars.The latest are about 2 yrs old. Although they are the softest ones so far, I have noticed a change. One nipple is lower than the other now. I know I was put on Accolate the first month after surgery and took it for 6 months. I recently saw a doctor that recommended having them taken out and clean out any scar tissue and before putting them back in he suggested injecting depomedrol in the capsule. I requested to use the same implants provided they are in good shape considering they are only 2 yrs old. Have you heard of this depomedrol technique and does it work? Jackie A: Hi Jackie, Depo-Medrol is a anti-inflammatory glucocorticoid which is not proven to do anything for capsular contracture. Personally I do not like steroid injections, especially those in the cortisone family, as they can have unwanted side effects. I do not think this ill be dangerous to try, but I would not place too much hope in it doing anything valuable to really help in the long run. My first consult for breast augmentation defined my breasts as "atrophied" (i.e.; deflated, nipples don't point downward or anything)...he went on to say that my condition is a "surgeon's test question" in that my implant should be placed over my muscle. His reasoning was that the location of my pec muscle in relation to that of my nipple would result in an unsightly "double boob" (at the top) appearance. I never knew there even was such a thing! He even went so far as to say over the muscle or not doing it at all were my only options.My second consult believed the above advice is nonsense and that I definitely could go under the muscle.My third consult said I could have my choice of going under or over.So my question is: Is there such a condition as what the 1st doctor claimed? Or could this have been a sales pitch of some sort? According to him, (my tissue thickness is about 1 1/2cm) you wouldn't be able to see modest-sized silicone implants. Glandular tissue will not shrink with weight loss, while fatty tissue will be absorbed This can usually be ascertained by your doctor to a relatively accurate degree. As far as how you may feel with smaller breasts and a tummy which remains a "work in progress", that is really something for you to consider carefully.
